Care Coordination/CaseManagement and NAOMI's Story

Naomi is a busy mother of 4 – three sons and a little daughter – and a household that is always buzzing with activity. In addition to her nuclear family, she has also taken in two young nieces in need, as well as Naomi’s mother who requires medical home care. 

This well-run home was Naomi’s dream, and a goal attainable with help from the Villa of Hope Adult Health Homes program. Naomi’s journey was not easy. In her own words, she started out “as a young single mother with a learning disability,” trying to navigate systems that were overwhelming. Additionally, she was apprehensive about talking to people, meeting strangers, and trusting those who offered help. It was a Villa Care Manager 4 years ago who stepped in and made her comfortable; helped her to see that she had a whole team behind her. “I was not afraid anymore,” she says, “and they broke problems down into steps I could accomplish at my own pace.”

While caring for her own children and her mother, Naomi worked with the Villa to connect with the First-time Home-buyer Program. On her own, Naomi built her credit, and saved for a down-payment on a house. At the age of 29, she became a proud homeowner.

“Naomi is amazing,” says Linda Kausch, her Villa Care Manager. “She has a heart of gold. She is strong, focused and intelligent, and I am simply her support.”

Her spacious 5-bedroom home is a haven for her own children and her Mom. She is currently also providing shelter for nieces who otherwise would have ended up in the foster care system. There is a big yard and a picnic table; and lots of smiles.

“The people I’ve worked with at the Villa are patient and polite,” Naomi says, “and they have motivated me to become who I am now. They always tell me to take my time, and that I’m doing a great job. 

“The Villa is my second family. I would recommend the Villa to anyone!”
